Insurance Information
In our office we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this our team works hand-in-hand with you to maximize your insurance reimbursement for covered procedures. At your consultation you will receive an estimate on how your insurance may help cover any treatment you need. The estimates are based on the information provided by your insurance company.
As a courtesy, we will gladly submit claims on your behalf to your insurance company regarding your treatment, however, we cannot guarantee benefit coverage. Please remember your estimated patient portion must be paid at the time of service and any uncovered benefits are your responsibility.
If you would like, we will be happy to submit a pre-authorization to your insurance company prior to beginning treatment. Please note that it may take 4-6 weeks to receive predeterminations back.
If your insurance plan is based on a fee schedule, it is very helpful if you bring in your insurance company’s most current fee schedule sheet so we can give you a more accurate estimate.

We accept all (private) dental insurance, however, we are ONLY preferred providers with:

Insurance NOT Accepted
- Apple Health
- Molina
- Medicaid
- Medicare
